By signing this form, you agree to adhere to the following terms:

Confidentiality and Professional Boundaries: You must maintain client confidentiality. Sharing personal contact information, such as phone numbers, with clients is strictly prohibited to protect professional boundaries.
Job Offers from Clients: Under no circumstances may you accept any job offers directly from clients. If found engaging in direct job solicitations, you will be barred from any future jobs with us, and unpaid wages will not be issued. Legal action may also be taken against you.
Work Assignment and Leave: You are expected to work according to the assignments provided by us. If you wish to take a day off, you must notify us at least one day in advance. If assigned to a permanent client, the required notice period is three days.
Automatic Contract Renewal: If you wish to cancel the contract, notify us 14 days in advance, or it will automatically renew.

Mga Tuntunin at Kundisyon

Sa pagpirma sa form na ito, sumasang-ayon ka sa mga sumusunod na tuntunin:

Kumpidensyalidad at Propesyonal na Hangganan: Dapat mong panatilihin ang kumpidensyalidad ng kliyente. Mahigpit na ipinagbabawal ang pagbabahagi ng personal na impormasyon tulad ng numero ng telepono sa mga kliyente upang mapanatili ang propesyonal na hangganan.
Mga Alok ng Trabaho mula sa Kliyente: Sa anumang pagkakataon, hindi ka maaaring tumanggap ng direktang alok ng trabaho mula sa mga kliyente. Kung mahuhuli kang nakikipag-ugnayan sa kliyente para sa trabaho, hindi ka na bibigyan ng anumang trabaho mula sa amin, at hindi ibibigay ang natitirang bayad sa sahod. Maari rin kaming magsagawa ng legal na aksyon laban sa iyo.
Pagtatalaga ng Trabaho at Paghingi ng Araw ng Pahinga: Inaasahan kang gagampanan ang trabaho ayon sa mga pagtatalaga na ibinibigay namin. Kung nais mong magpahinga ng isang araw, kailangang ipaalam mo ito ng hindi bababa sa isang araw bago ang araw ng pahinga. Kung nakatalaga ka sa isang permanenteng kliyente, ang kinakailangang panahon ng abiso ay tatlong araw.
Awtomatikong Pag-renew ng Kontrata: Kung nais mong tapusin ang kontrata, kailangang ipaalam ito sa amin ng hindi bababa sa 14 na araw bago ang petsa ng kanselasyon, o awtomatikong marerenew ang kontrata.
